Enhancing Antiplatelet Management in Carotid Artery Stenting with Impedance Aggregometry - Multiplate® Analysis

A recent retrospective study in Clinics and Practice highlights a key challenge in carotid artery stenting (CAS): many patients on standard dual antiplatelet therapy exhibit high on-treatment platelet reactivity (HTPR) — meaning the medication isn’t adequately inhibiting platelet activity, a risk factor for post-procedural thrombotic events.


What’s especially insightful is the role of Multiplate® platelet function analysis in identifying this issue. The study found:


✔️ ~31% of patients on clopidogrel showed HTPR when assessed with Multiplate®


 ✔️ Alternative P2Y12 inhibitors (like ticagrelor and ticlopidine) consistently normalized platelet reactivity in those patients


 ✔️ No residual HTPR was seen in patients switched based on Multiplate® results
